Saw palmetto (Serenoa repens) berry extract inhibits 5-alpha reductase, the enzyme that converts testosterone to dihydrotestosterone (DHT), making it the most studied herbal treatment for benign prostatic hyperplasia (BPH). Meta-analyses show improvements in urinary flow rate, frequency, and nocturia. While some large trials have questioned its efficacy, liposterolic extracts standardized to 85-95% fatty acids (at 320mg/day) show the most consistent clinical benefit.
